As a Social Worker in our LD Team, you will be responsible for:

  • Conducting comprehensive assessments to identify the needs and strengths of individuals with learning disabilities.
  • Developing and implementing personalized support plans in collaboration with service users and their families.
  • Providing ongoing support and advocacy to individuals, ensuring they have access to the resources and services they require.
  • Working closely with other professionals, including health services, to coordinate care and support.
  • Monitoring and reviewing care plans to ensure they remain effective and relevant to the individual's needs.

To be considered for this role, candidates must possess the following essential qualifications and skills:

  • A recognized Social Work qualification (e.g., BA/MA in Social Work).
  • Registered with Social Work England.
  • Experience working with adults with learning disabilities or related fields.
  • Strong assessment and analytical skills, with the ability to develop person-centered support plans.
  • Excellent communication and interpersonal skills, with a commitment to advocacy and empowerment.
