In this role, you will be responsible for a range of essential duties, including:
Assessing the needs of adults with learning disabilities and developing tailored support plans. Working collaboratively with clients, families, and other professionals to facilitate access to services. Monitoring and reviewing care plans to ensure they meet the evolving needs of clients. Providing advocacy and support to clients to promote their rights and choices. Maintaining accurate and timely documentation in compliance with organizational standards.To be considered for this position, you must possess the following essential qualifications and skills:
Qualified Social Worker status with a relevant degree in social work. Registration with Social Work England. Proven experience working with adults with learning disabilities. Strong interpersonal and communication skills, with the ability to build rapport with clients.
