We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's SW - Neighbourhood Mental Health & Learning Disabilities Team in Somerset. This role offers an exciting opportunity to make a meaningful impact in the lives of adults with mental health challenges and learning disabilities. As part of our dynamic team, you will work collaboratively with service users, their families, and multidisciplinary professionals to promote well-being and independence.

Key responsibilities include:

  • Conducting comprehensive assessments of service users' needs and developing tailored support plans.
  • Providing ongoing support and intervention to individuals with mental health issues and learning disabilities.
  • Collaborating with healthcare providers, community organizations, and families to ensure holistic care.
  • Advocating for service users' rights and facilitating access to necessary resources and services.
  • Maintaining accurate records and documentation in compliance with regulatory standards.

To be successful in this role, candidates must possess the following qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work or equivalent).
  • Registration with Social Work England.
  • Proven experience in adult social work, particularly in mental health and learning disabilities.
  • Strong assessment and intervention skills, with the ability to work under pressure.
  • Excellent communication and interpersonal skills, with a compassionate approach to service users.
