Your key responsibilities will include:

  • Conducting comprehensive assessments of individuals’ needs and developing personalized support plans.
  • Providing direct support and guidance to individuals and their families, promoting independence and social inclusion.
  • Collaborating with multidisciplinary teams and external agencies to coordinate effective services.
  • Monitoring and reviewing care plans to ensure the highest standards of care are maintained.

To be successful in this role, you will need the following essential qualifications and skills:

  • A relevant social care qualification (e.g., Degree in Social Work, Diploma in Social Work, or equivalent).
  • Experience working with adults with learning disabilities and autism.
  • Strong communication and interpersonal skills, with the ability to build rapport with clients and their families.
  • Knowledge of relevant legislation and best practices in social care.
  • Ability to work effectively in a team and manage a caseload efficiently.
