In this rewarding role, you will provide essential support to individuals with learning disabilities, ensuring they lead fulfilling lives within their communities. You will work closely with clients, families, and multidisciplinary teams to develop tailored care plans that promote independence and well-being.
Your key responsibilities will include:
Conducting comprehensive assessments to identify the needs and strengths of clients. Creating and implementing individualized support plans in collaboration with clients and their families. Providing ongoing support and guidance to clients, helping them navigate services and access resources. Advocating for clients' rights and needs within various settings, including healthcare and educational institutions. Maintaining accurate and up-to-date case records and documentation in compliance with regulatory standards.To be considered for this role, you must possess the following qualifications and skills:
A recognized social work qualification (Degree in Social Work or equivalent). Registration with Social Work England. Experience working with individuals with learning disabilities and a solid understanding of relevant legislation. Excellent interpersonal and communication skills, with the ability to build rapport and trust with clients.
