Join our dedicated Adult's Social Work - Learning Disabilities (LD) Team in Somerset, where your skills and compassion can make a real difference in the lives of individuals with learning disabilities. We are seeking a qualified and motivated Social Worker to provide high-quality support and intervention to adults, ensuring their needs are met and their voices are heard. This role offers a unique opportunity to work collaboratively with individuals, families, and multi-disciplinary teams to promote independence and well-being.

As a Social Worker in our LD Team, you will be responsible for:

Conducting comprehensive assessments to identify the needs and strengths of individuals with learning disabilities. Developing and implementing personalized support plans in collaboration with service users and their families. Providing ongoing support and advocacy to individuals, ensuring they have access to the resources and services they require. Working closely with other professionals, including health services, to coordinate care and support. Monitoring and reviewing care plans to ensure they remain effective and relevant to the individual's needs.

To be considered for this role, candidates must possess the following essential qualifications and skills:

A recognized Social Work qualification (e.g., BA/MA in Social Work). Registered with Social Work England. Experience working with adults with learning disabilities or related fields. Strong assessment and analytical skills, with the ability to develop person-centered support plans. Excellent communication and interpersonal skills, with a commitment to advocacy and empowerment.

We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's SW - Neighbourhood Mental Health & Learning Disabilities Team in Somerset. This role offers an exciting opportunity to make a meaningful impact in the lives of adults with mental health challenges and learning disabilities. As part of our dynamic team, you will work collaboratively with service users, their families, and multidisciplinary professionals to promote well-being and independence.

Key responsibilities include:

Conducting comprehensive assessments of service users' needs and developing tailored support plans. Providing ongoing support and intervention to individuals with mental health issues and learning disabilities. Collaborating with healthcare providers, community organizations, and families to ensure holistic care.
