Birmingham - Social Worker - Transitions Team

- Conducting assessments and developing tailored care plans for young individuals in transition.

- Monitoring progress and adjusting care plans as necessary to meet the evolving needs of each individual.
- Maintaining accurate records and documentation in compliance with local and national regulations.
To be considered for this role, you must possess the following qualifications and skills:
- A recognized social work qualification (e.g., BA/MA in Social Work).
- Current Social Work England registration.
- Experience working with young people, particularly in a transitions or leaving care context.
- Strong communication and interpersonal skills, with the ability to build rapport with young individuals and their families.
- Excellent organizational skills and the ability to manage a varied caseload effectively.
