We are seeking a dedicated and compassionate Social Worker L3 Sass Ca11 - 1157 Grade K to join our dynamic team in the Qualified Social Work field. In this role, you will be responsible for providing high-quality social work services to individuals and families facing various challenges. You will work collaboratively with clients to assess their needs, develop personalized care plans, and facilitate access to necessary resources and support systems.
Your key responsibilities will include:
Conducting comprehensive assessments to identify client needs and strengths. Developing and implementing individualized care plans in collaboration with clients and multidisciplinary teams. Providing counseling and support to clients and their families, addressing emotional, social, and financial issues. Advocating for clients’ rights and connecting them with community resources and services. Maintaining accurate and up-to-date documentation of client interactions and progress.To be successful in this role, you should possess the following essential qualifications and skills:
A Bachelor’s degree in Social Work (BSW) or a related field; Master’s degree preferred. Current licensure as a social worker in your state. Proven experience in social work, particularly in case management and client advocacy. Strong interpersonal and communication skills, with the ability to build rapport with diverse populations.
