Join our dedicated team at East Riding as a Social Worker in the 1-1 Project Team within Adult Social Care. In this rewarding role, you will provide personalized support to adults, helping them achieve their goals and enhance their quality of life. You will work closely with individuals, families, and multidisciplinary teams to assess needs, develop care plans, and deliver tailored interventions that empower clients to live independently.

Your key responsibilities will include:

Conducting comprehensive assessments of clients to identify their needs and preferences. Developing and implementing care plans that promote independence and well-being. Collaborating with healthcare professionals, community services, and families to ensure holistic support. Monitoring and reviewing care plans regularly to adapt to changing needs. Maintaining accurate and up-to-date case records in accordance with legal and organizational standards.

The ideal candidate will possess the following essential qualifications and skills:

A recognized Social Work qualification (e.g., Degree in Social Work, CQSW, or DipSW). Registration with Social Work England. Experience in adult social care, particularly in one-to-one support settings. Strong communication and interpersonal skills, with the ability to build rapport with clients. Excellent problem-solving skills and the ability to work independently and as part of a team.
