Adult's SW - LD & PD Team - City of Wolverhampton

We are currently seeking a dedicated and experienced Adult's Social Worker to join our Learning Disabilities and Physical Disabilities Team in the City of Wolverhampton. As a key member of the team, you will be responsible for providing high-quality social work support to adults with learning disabilities and physical disabilities, ensuring their well-being and promoting their independence.
Key responsibilities and duties:
- Assessing the needs of individuals with learning disabilities and physical disabilities, and developing care plans tailored to their specific requirements.
- Working collaboratively with multi-disciplinary teams, service users, and their families to ensure holistic and person-centered care.
- Providing advocacy and support to empower individuals to make informed decisions about their care and support needs.
- Conducting safeguarding investigations and interventions to protect vulnerable adults from harm or abuse.
- Participating in case conferences, reviews, and court proceedings as required, and maintaining accurate and up-to-date case records.
Essential qualifications and skills:
- Qualified Social Worker with a relevant social work degree and HCPC registration.
- Demonstrated experience working with adults with learning disabilities and physical disabilities in a social work setting.
- Strong assessment, communication, and interpersonal skills, with the ability to build positive relationships with service users and stakeholders.
- Sound knowledge of relevant legislation, policies, and procedures related to adult social care and safeguarding.
- Ability to work autonomously, manage a complex caseload, and prioritize tasks effectively.
