We are currently seeking a dedicated Adult's Social Worker to join our Assessment Team in Sefton. As a member of our team, you will play a crucial role in assessing the needs of vulnerable adults and developing personalized care plans to support their well-being.

Key responsibilities and duties:

Conducting thorough assessments of adults in need of support and intervention Collaborating with multi-disciplinary teams to develop and implement care plans Providing support and guidance to individuals and their families to promote independence and well-being Monitoring and reviewing care plans to ensure they are meeting the needs of service users Participating in safeguarding procedures and taking appropriate action when necessary

Essential qualifications and skills:

Qualified Social Worker with current registration with Social Work England Experience working with vulnerable adults in a social care setting Strong assessment and analytical skills Excellent communication and interpersonal skills Ability to work effectively in a fast-paced and challenging environment

Experience working in a similar role within an Assessment Team is highly desirable.
