We are currently seeking a dedicated and experienced Adult's Social Worker to join our Locality Team in Liverpool. As a key member of the team, you will be responsible for providing high-quality social work services to vulnerable adults within the community.
Key responsibilities and duties:
Conducting assessments and developing care plans for adults in need of support Working collaboratively with other professionals and agencies to ensure holistic care for service users Providing advocacy and support to empower individuals to make informed decisions about their care Participating in case conferences and reviews to monitor progress and adjust care plans as needed Ensuring compliance with relevant legislation and best practice guidelines in all aspects of social workEssential qualifications and skills:
Qualified Social Worker with a valid HCPC registration Demonstrated experience in adult social work, including assessments and safeguarding Strong communication and interpersonal skills to engage effectively with service users and their families Ability to work autonomously and as part of a multidisciplinary team Commitment to ongoing professional development and upholding ethical standards in social work practiceAdditional requirements or benefits:

We are currently seeking a dedicated Adult's Social Worker to join our Locality Team in Liverpool. As a key member of our team, you will be responsible for providing high-quality social work services to adults in need of support and intervention.
Key responsibilities and duties include:
Conducting assessments to determine the needs and risks of adults in the community Developing and implementing care plans to address the identified needs of service users Working collaboratively with multi-disciplinary teams and partner agencies to provide holistic support Participating in safeguarding investigations and taking appropriate action to protect vulnerable adults Advocating for service users to ensure their voices are heard and their rights are upheldThe ideal candidate for this role will possess:
A recognized Social Work qualification and registration with Social Work England Demonstrated experience in adult social work, preferably within a locality team setting Strong assessment and analytical skills to effectively identify needs and risks Excellent communication and interpersonal skills to engage with service users, families, and stakeholders Ability to work autonomously and as part of a team in a fast-paced environmentIn addition to the essential qualifications and skills, candidates with experience in mental health, learning disabilities, or older adults' services will be highly valued.
