We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work Hospital Team in the London Borough of Lambeth. Key responsibilities and duties include:

  • Conducting comprehensive assessments of individuals’ needs and risks in collaboration with healthcare professionals.
  • Developing and implementing person-centered care plans that promote independence and well-being.
  • Providing information, advice, and support to individuals and their families regarding available services and resources.
  • Facilitating safe discharges from hospital, coordinating with community services and other stakeholders.
  • Maintaining accurate and up-to-date case records in line with statutory requirements and organizational policies.

The ideal candidate will possess the following essential qualifications and skills:

  • A degree in Social Work and registration with Social Work England.
  • Proven experience in adult social work, particularly in a hospital or health care setting.
  • Strong assessment and analytical skills, with the ability to make informed decisions under pressure.
  • Excellent communication and interpersonal skills, with the ability to build rapport with diverse individuals.
  • Knowledge of relevant legislation and best practices in social care.
