Key responsibilities include:
Conducting comprehensive assessments of patients' social, emotional, and practical needs prior to discharge. Collaborating with multidisciplinary teams to develop tailored discharge plans that promote recovery and well-being. Providing ongoing support and guidance to patients and their families during the discharge process. Connecting patients with community resources and services to facilitate a successful transition. Maintaining accurate records and documentation in accordance with organizational policies and legal requirements.To be successful in this role, candidates must possess the following qualifications and skills:
A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registered with Social Work England (SWE). Experience working in mental health settings, particularly within hospital discharge processes. Excellent communication and interpersonal skills, with the ability to build rapport with patients and their families.
