We are seeking a dedicated and experienced Senior Social Worker to join our Hospital Discharge (Out of Borough) Team in Croydon. In this pivotal role, you will be responsible for facilitating safe and timely discharges for patients, ensuring their ongoing support and well-being in the community. You will work collaboratively with healthcare professionals, patients, and their families to assess needs, develop discharge plans, and connect individuals with appropriate services.

Your key responsibilities will include:

  • Conducting comprehensive assessments of patients’ social care needs upon hospital admission and prior to discharge.
  • Developing and implementing discharge plans that ensure continuity of care and support.
  • Collaborating with multidisciplinary teams to coordinate care pathways and address any barriers to discharge.
  • Providing guidance and support to junior social workers and other team members.
  • Maintaining accurate and timely documentation in accordance with legal and organizational standards.

To be considered for this role, you must possess the following essential qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work, CQSW, or equivalent).
  • Current registration with Social Work England.
  • Extensive experience in adult social care, particularly in hospital discharge settings.
  • Strong assessment and analytical skills, with the ability to work effectively under pressure.
  • Excellent communication and interpersonal skills for engaging with diverse stakeholders.
