As a Social Worker in the Adults - Transformation team, your key responsibilities will include:

Conducting comprehensive assessments to identify the needs and strengths of individuals. Developing and implementing tailored care plans in collaboration with service users and their families. Providing ongoing support and guidance to individuals, ensuring they have access to necessary resources and services. Working closely with multi-disciplinary teams and external agencies to advocate for the best interests of service users. Monitoring and reviewing care plans to ensure effectiveness and making adjustments as needed. Maintaining accurate and up-to-date case records in accordance with legal and organizational standards.

To be successful in this role, you must possess the following essential qualifications and skills:

A recognized Social Work qualification (e.g., BA/MA in Social Work). Current registration with Social Work England. Proven experience working with adults in a social work capacity. Strong assessment and analytical skills with the ability to develop effective care plans. Excellent communication and interpersonal skills, with a compassionate approach to service users. Ability to work collaboratively within a team and manage a diverse caseload.
