- Ensure support plans clearly reflect individuals' needs and outcomes, regardless of eligibility decisions.

- Assess and provide advice on minor and major adaptations to properties.
- Assess and advise on moving and handling techniques, including postural management support and appropriate equipment.
- Work closely with housing partners to support re-housing applications in line with local housing policy.
Safeguarding, Risk and Professional Practice
- Act as a trusted assessor, identifying when specialist assessment or intervention from another professional is required.
- Identify, report and progress adult and children safeguarding concerns, carrying out safeguarding enquiries in line with local procedures and the principles of Making Safeguarding Personal.
- Apply legal and professional judgement to balance protection and risk, promoting positive risk management.
- Maintain comprehensive, accurate and professionally defensible records in line with organisational and professional standards.
- Where required, write court reports and provide evidence in legal proceedings.
Additional Responsibilities
- Work in accordance with departmental policies and procedures, including GDPR, data protection, confidentiality and information-sharing protocols.
- Take responsibility for delivering against individual and team performance targets.
- Be accountable for the commitment of financial resources within the agreed scheme of delegation.
- Apply principles of social justice, social inclusion and equality in all casework, actively challenging discrimination and oppression and demonstrating sound ethical reasoning.
