We are currently seeking a passionate and dedicated Conwy Qualified Social Worker (QSW) with experience in working with individuals with disabilities aged 0-25. As part of this role, you will be responsible for providing high-quality social work services to support and empower individuals within this age group.

Key responsibilities and duties:

  • Assessing the needs of individuals with disabilities aged 0-25 and developing care plans tailored to their specific requirements.
  • Working collaboratively with other professionals, such as healthcare providers and educators, to ensure a holistic approach to care and support.
  • Advocating for the rights and best interests of the individuals you work with, ensuring their voices are heard and respected.
  • Providing support to families and caregivers, offering guidance and resources to help them better care for their loved ones.

  • Essential qualifications and skills:

    • Qualified Social Worker (QSW) with a valid registration with the relevant regulatory body.
    • Demonstrated experience in working with individuals with disabilities, preferably within the 0-25 age group.
    • Strong assessment and case management skills, with the ability to develop and implement effective care plans.
    • Excellent communication and interpersonal skills, with the ability to build rapport with clients and collaborate with colleagues.
    • Knowledge of relevant legislation and policies related to social work practice with individuals with disabilities.
