We are currently seeking a dedicated and passionate Adult's Social Worker to join our Mental Health Team in North Lincolnshire. As a key member of the team, you will be responsible for providing high-quality social work services to adults with mental health needs, ensuring their well-being and promoting their independence.

Key responsibilities and duties:

Conducting assessments and developing care plans for individuals with mental health issues Working collaboratively with multidisciplinary teams to provide holistic support Advocating for clients and ensuring their voices are heard in decision-making processes Monitoring and reviewing the effectiveness of interventions and making necessary adjustments Providing crisis intervention and support during times of acute mental health distress

Essential qualifications and skills:

Qualified Social Worker with current registration with HCPC Demonstrated experience working with adults with mental health needs Strong assessment and care planning skills Excellent communication and interpersonal skills Ability to work effectively under pressure and manage complex cases
