Join our dedicated team in Conwy as a Social Worker specializing in Adults with Disabilities. You will work collaboratively with service users, their families, and multidisciplinary teams to deliver high-quality assessments and interventions.
Your key responsibilities will include:
Conducting comprehensive assessments of individuals’ needs and developing tailored support plans. Providing ongoing support and guidance to service users and their families, advocating for their rights and needs. Coordinating with healthcare professionals, community services, and other stakeholders to ensure holistic care. Monitoring and reviewing care plans to ensure they remain relevant and effective. Maintaining accurate and up-to-date documentation in accordance with regulatory standards.To be successful in this role, you will need:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current registration with Social Care Wales. Experience working with adults with disabilities or in a similar social work setting. Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
