Job Overview
We are seeking a dedicated and experienced Adult Social Worker to join our Community Learning Disabilities Transition Team. This role is vital in supporting young people with learning disabilities as they transition from children's services into adult social care, ensuring a seamless and person-centred approach to independence, well-being, and social inclusion.
Key Responsibilities
- Work collaboratively with young people, their families, carers, and multi-disciplinary teams to develop transition plans that promote independence and self-determination.
- Conduct strengths-based assessments and develop person-centred care and support plans in line with the Care Act 2014.
- Ensure smooth transitions from children's services to adult social care, providing advocacy and guidance throughout the process.
- Work in partnership with health professionals, education providers, and third-sector organizations to coordinate services and support.
- Apply knowledge of Mental Capacity Act 2005, Deprivation of Liberty Safeguards (DoLS), and Safeguarding Adults legislation to ensure individuals' rights and well-being.
- Monitor, review, and reassess care plans, making necessary adjustments to ensure evolving needs are met.
- Maintain accurate and up-to-date case records in line with local authority policies and statutory requirements.
- Attend team meetings, training, and supervision to ensure professional development and best practice adherence.
Essential Criteria
- Qualified Social Worker (Degree in Social Work, DipSW, CQSW, or equivalent).
- Registration with Social Work England.
- Demonstrated experience working with adults with learning disabilities and/or transition services.
- Strong understanding of Care Act 2014, Mental Capacity Act 2005, and safeguarding legislation.
- Ability to work independently and collaboratively within a multi-disciplinary setting.
- Excellent assessment, planning, and communication skills.
- Proficiency in IT systems and case management software.
- A valid UK driving licence (desirable for community-based work).

To coordinate the work of the Children Services and Adult Services in the transition of young people with social care needs as they enter adulthood.
The role will ensure that high quality and timely transition support is offered to young people and their carers in accordance with Care Act duties and the principals of Preparing for Adulthood.
The role has a specific focus on working with colleagues and external partners to develop effective transition arrangements for young people with the most significant needs.
The role will support engagement with Children and Young People's Health, Social Care and SEND services to support effective pathways for young people to develop their independence.
The role will support effective planning and procurement of local supported living services for young people with complex needs, offering high quality and cost-effective alternatives to out of area placements.
The role will work in collaboration with other Team Manager colleagues in the Learning Disability Team and Children with Disabilities Team as part of a combined management team.
Deputise for the Service Manager as required and keep them apprised of sensitive, high-profile issues.
Responsible for quality assurance and contribute to the strategic direction of the service.
