South Gloucestershire Council is seeking a dedicated and experienced Qualified Social Worker to join our Adult's Assessment Team. Your key responsibilities will include:

  • Conducting comprehensive assessments of individuals' needs and risks, ensuring that all relevant information is gathered and analyzed.
  • Developing and implementing person-centered care plans in collaboration with clients, families, and other professionals.
  • Providing ongoing support and review of care plans to ensure they meet the evolving needs of clients.
  • Maintaining accurate and timely documentation in accordance with statutory requirements and council policies.
  • Collaborating with multidisciplinary teams and external agencies to facilitate effective service delivery.

The ideal candidate will possess the following essential qualifications and skills:

  • A recognized Social Work qualification (BA/MA in Social Work or equivalent).
  • Registration with Social Work England.
  • Proven experience in adult social work, particularly within assessment and care planning.
  • Strong communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
  • Excellent organizational skills and the ability to manage a caseload effectively.
