Caritas Recruitment is currently seeking an experienced Mental Health Social Worker to join a dedicated Community Mental Health Team in Hampshire.
This is an excellent opportunity for a qualified Social Worker with experience in mental health services to work within a supportive multidisciplinary team, helping adults with complex mental health needs to achieve positive outcomes and maintain independence within their communities.
About the Role
As a Mental Health Social Worker, you will manage a varied caseload of adults with mental health needs, undertaking assessments, managing risk, and supporting individuals through strengths-based and person-centred practice.
You will work collaboratively with healthcare professionals, families, carers, and community partners to ensure service users receive the appropriate support and interventions.
Key Responsibilities
- Completing Care Act and mental health assessments
- Managing a complex caseload within the community
- Undertaking safeguarding enquiries and risk assessments
- Supporting service users with care planning and reviews
- Working within relevant legislation, including the Care Act, Mental Capacity Act, and Mental Health Act
- Completing home visits across the local area
- Liaising with partner agencies and multidisciplinary teams
- Promoting independence, wellbeing, and recovery-focused outcomes
Essential Requirements
- Qualified Social Worker with a recognised Social Work Degree
- Social Work England registration
- Previous experience within a Community Mental Health Team
- Working knowledge of the Care Act, Mental Capacity Act, and Mental Health Act
- Strong assessment and risk management skills
- Full UK driving licence and access to a vehicle
