The City of Westminster is seeking a dedicated and compassionate Adult's Social Worker to join our dynamic Hospital Team. In this role, you will play a crucial part in providing high-quality support to adults in need, working collaboratively with healthcare professionals and families to ensure the best outcomes for our clients. Your expertise will help navigate complex situations and promote the welfare of vulnerable adults within our community.
Key responsibilities include:
Conducting comprehensive assessments of individuals’ needs and risks, ensuring a person-centered approach. Developing and implementing care plans that address the identified needs of clients. Collaborating with multidisciplinary teams, including medical staff, to facilitate smooth transitions from hospital to home or other care settings. Providing emotional support and guidance to clients and their families throughout the care process. Maintaining accurate and up-to-date client records and documentation in compliance with legal and organizational standards.To be considered for this role, candidates must possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work or equivalent). Current registration with Social Work England. Proven experience in adult social work, particularly in a hospital or healthcare setting. Strong communication and interpersonal skills, with the ability to build rapport with clients and colleagues alike.
