Join our dedicated Hospital Discharge Team in Westminster as a Social Worker, where you will play a vital role in facilitating smooth transitions for patients moving from hospital to home or community care. In this dynamic position, you will work collaboratively with healthcare professionals, patients, and their families to ensure that all discharge planning is comprehensive, person-centered, and meets the diverse needs of our clients.
Your key responsibilities will include:
Conducting assessments to identify the social, emotional, and practical needs of patients prior to discharge. Developing and implementing individualized discharge plans in collaboration with multidisciplinary teams. Providing support and guidance to patients and families, ensuring they understand the discharge process and available resources. Coordinating with community services and agencies to facilitate post-discharge care and support. Documenting all assessments, plans, and interventions in accordance with legal and organizational standards.The ideal candidate will possess a recognized social work qualification (CQSW, DipSW, or equivalent) and be registered with Social Work England. You should have a solid understanding of hospital discharge processes and relevant legislation, along with experience in working within a healthcare setting. Key skills include strong communication and interpersonal abilities, as well as effective problem-solving and organizational skills.
