We are currently seeking a dedicated and experienced Qualified Social Worker to join our Transitions Team in Wolverhampton. As a key member of the team, you will be responsible for providing support and guidance to young adults transitioning from care to independent living.

Key responsibilities and duties include:

  • Conducting assessments and developing care plans for young adults aged 16-25.
  • Supporting individuals in developing essential life skills, such as budgeting, cooking, and accessing education or employment opportunities.
  • Liaising with external agencies and professionals to ensure a holistic approach to support and care.
  • Advocating for the rights and well-being of young adults in the transition process.

The ideal candidate will possess the following essential qualifications and skills:

  • Qualified Social Worker with relevant experience in working with young adults in a transitions or leaving care setting.
  • Registration with Social Work England and a valid DBS check.
  • Excellent communication and interpersonal skills to build positive relationships with service users and stakeholders.
  • Strong assessment and care planning abilities to meet the diverse needs of young adults.

In addition to the above requirements, candidates with experience in supporting individuals with complex needs, such as mental health challenges or disabilities, will be highly valued.
