We are currently seeking a dedicated and experienced Adult's Social Worker to join our Assessment Team in Bournemouth. As a key member of the team, you will be responsible for conducting comprehensive assessments of adults in need of support and services, ensuring their well-being and safety.

Key responsibilities and duties include:

Undertaking holistic assessments to identify the needs and risks of vulnerable adults Developing and implementing care plans in collaboration with service users, families, and multi-disciplinary teams Providing support and guidance to individuals to promote independence and enhance their quality of life Participating in case conferences, reviews, and safeguarding meetings Maintaining accurate and up-to-date records in accordance with regulatory standards

The ideal candidate for this role will possess the following essential qualifications and skills:

Qualified Social Worker with a valid HCPC registration Demonstrated experience in conducting assessments and interventions with adults in a social care setting Strong communication and interpersonal skills to engage effectively with service users and stakeholders Ability to work autonomously and as part of a team in a fast-paced environment Sound knowledge of relevant legislation, policies, and procedures in adult social care
