Your key responsibilities will include:
Conducting thorough assessments of individuals' needs and risks. Developing and implementing tailored care plans in collaboration with service users and their families. Working closely with multi-disciplinary teams and external agencies to coordinate support services. Providing expert advice and support to colleagues and stakeholders on safeguarding matters. Maintaining accurate and up-to-date case records and documentation in compliance with legal and organizational standards.The ideal candidate will possess the following essential qualifications and skills:
A relevant social work qualification (Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Proven experience in adult safeguarding and social work practice. Strong assessment and analytical skills.
