We are currently seeking a dedicated and experienced Qualified Social Worker to join our Children's Disability Service team in Blackburn. As a key member of the team, you will be responsible for providing high-quality support and intervention to children and families with disabilities, ensuring their well-being and promoting their independence.

Key responsibilities and duties:

Assessing the needs of children with disabilities and developing care plans in collaboration with the child, family, and other professionals Providing direct support and intervention to children and families, including advocacy and guidance on accessing appropriate services Monitoring and reviewing the progress of children and families, adjusting care plans as needed to ensure positive outcomes Participating in multi-disciplinary team meetings and case conferences to coordinate care and support services

Essential qualifications and skills:

Qualified Social Worker with a valid HCPC registration Experience working in a similar role within children's services, preferably with a focus on disabilities Strong assessment and intervention skills, with the ability to work collaboratively with children and families Excellent communication and interpersonal skills, with the ability to build rapport and trust with diverse populations

Responding to referrals and conducting initial assessments to determine the level of support required Undertaking comprehensive assessments and developing care plans for children in need, including those in need of protection Working collaboratively with multi-disciplinary teams and external agencies to ensure the safety and well-being of children Participating in case conferences, court proceedings, and reviews to advocate for the best interests of children Providing support and guidance to families to promote positive outcomes and prevent future safeguarding concerns

Essential qualifications and skills:
