We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work team within a leading hospital in Warrington. This role is essential in providing high-quality support and intervention for adults with complex needs, ensuring their well-being and enhancing their quality of life. Key responsibilities include:

  • Conducting comprehensive assessments of individuals' needs and circumstances.
  • Developing, implementing, and reviewing care plans in partnership with service users and their families.
  • Providing emotional support and guidance to clients and their families during challenging times.
  • Collaborating with multidisciplinary teams to facilitate effective treatment and discharge planning.
  • Advocating for clients’ rights and ensuring access to necessary services and resources.
  • Maintaining accurate and up-to-date case records in accordance with legal and organizational standards.

To be considered for this position, you must possess the following qualifications and skills:

  • A relevant degree in Social Work and current registration with Social Work England.
  • Demonstrated experience in adult social work, preferably within a hospital setting.
  • Strong assessment and intervention skills with a focus on safeguarding and risk management.
  • Excellent communication and interpersonal skills, with the ability to build rapport with diverse individuals.
  • Proficient in case management software and Microsoft Office applications.
