We are seeking a dedicated and experienced Social Worker to join our Assessment Team in Reading, focusing on adult services. This role is crucial in providing high-quality assessments and support to vulnerable adults in our community. As a key member of our team, you will be responsible for conducting comprehensive assessments, developing care plans, and ensuring that individuals receive the appropriate services tailored to their needs.

Your key responsibilities will include:

  • Conducting initial assessments and ongoing reviews of adult clients’ needs.
  • Collaborating with multidisciplinary teams to ensure holistic care and support.
  • Developing and implementing person-centered care plans in line with best practices.
  • Providing guidance and support to clients and their families, advocating for their rights and needs.
  • Maintaining accurate and up-to-date case records in compliance with relevant legislation and policies.

The ideal candidate will possess the following essential qualifications and skills:

  • A recognized social work qualification (e.g., BA/MA in Social Work, CQSW, DipSW).
  • Current registration with Social Work England.
  • Proven experience in adult social work, particularly in assessment and care planning.
  • Strong communication and interpersonal skills, with the ability to build rapport with clients.
  • Knowledge of relevant legislation, policies, and procedures in adult social care.
