We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work - Discharge to Assess (D2A) team in Essex. As a vital part of our organization, you will play a crucial role in supporting adults in transitioning from hospital to home, ensuring their safety, well-being, and a smooth reintegration into the community. Key responsibilities include:
Conducting comprehensive assessments of individuals’ needs and circumstances to inform care planning. Collaborating with healthcare professionals, families, and community organizations to facilitate effective discharge planning. Implementing and reviewing care plans, ensuring they are tailored to the individual’s requirements. Providing ongoing support and guidance to individuals and their families throughout the transition process. Maintaining accurate and up-to-date records and documentation in accordance with organizational policies and procedures.To be considered for this role, you must possess the following essential qualifications and skills:
A recognized social work qualification (e.g., BA/MA in Social Work, DipSW, or equivalent). Registration with Social Work England. Experience working in adult social care, particularly within a hospital or D2A setting. Strong assessment and analytical skills, with the ability to make informed decisions.
