We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work - Hospital Team in Stoke on Trent. This is an exciting opportunity to make a significant impact on the lives of individuals in need of support during their hospital stay and transition back to the community. You will work collaboratively with healthcare professionals, family members, and other stakeholders to ensure that patients receive the best possible care and support.

Key responsibilities and duties include:

  • Conducting assessments of patients' social care needs and developing tailored care plans.
  • Providing emotional and practical support to patients and their families.
  • Collaborating with multidisciplinary teams to facilitate safe discharges and ongoing support.
  • Advocating for patients' rights and ensuring they have access to necessary services.
  • Maintaining accurate and up-to-date records in line with organizational policies and procedures.

The ideal candidate will possess the following essential qualifications and skills:

  • A recognized Social Work qualification (e.g., BA/MA in Social Work).
  • Registered with Social Work England.
  • Strong knowledge of adult safeguarding, mental health legislation, and community resources.
  • Excellent communication and interpersonal skills, with the ability to engage effectively with diverse populations.
  • Proven ability to work collaboratively in a fast-paced environment.
