Join our dedicated Bradford - AP - Locality Team as a Qualified Social Worker, where you will play a vital role in supporting individuals and families within the community. Your primary responsibility will be to assess and manage cases, ensuring that vulnerable populations receive the care and support they need. You will collaborate with multidisciplinary teams to develop and implement effective care plans, advocate for service users, and promote their welfare and rights.

Key responsibilities include:

Conducting comprehensive assessments of service users’ needs and circumstances. Developing and reviewing care plans in collaboration with service users, families, and other professionals. Providing support and guidance to individuals and families, ensuring they are empowered to make informed decisions. Maintaining accurate and up-to-date case records in accordance with local policies and procedures. Participating in team meetings and contributing to service development initiatives.

To be considered for this role, you must possess the following essential qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work or equivalent). Registration with Social Work England. Significant experience in social work, particularly within a locality team setting. Strong assessment and analytical skills, with the ability to manage complex cases. Excellent communication and interpersonal skills, enabling effective collaboration with diverse stakeholders.
