Are you a dedicated and experienced Advanced Practitioner looking to make a significant impact in the AHP field? Staffordshire's Front Door Team is seeking a passionate individual to join us in delivering high-quality services and support to our community. As an Advanced Practitioner, you will play a crucial role in assessing needs, providing expert guidance, and ensuring effective service delivery within a multi-disciplinary environment.
Your key responsibilities will include:
Conducting comprehensive assessments and developing tailored care plans for service users. Providing expert advice and support to team members, ensuring best practices are followed. Collaborating with various stakeholders, including health professionals, social services, and community organizations. Leading case reviews and contributing to the continuous improvement of services. Staying updated on legislation and policies relevant to the AHP field.To be successful in this role, you will need the following essential qualifications and skills:
A relevant degree in a health or social care discipline, along with post-qualification experience. Registration with a relevant professional body (e.g., HCPC). Strong assessment, analytical, and problem-solving skills. Excellent communication and interpersonal abilities, with a commitment to collaborative working.
