Join our dedicated Localities Team in Stoke on Trent as a Qualified Social Worker specializing in Adult Services. Your role will involve conducting assessments, developing care plans, and collaborating with various stakeholders to deliver high-quality services.
Your key responsibilities will include:
Conducting comprehensive assessments of clients’ needs and circumstances. Developing and implementing personalized care plans in partnership with clients and their families. Providing ongoing support and monitoring progress to ensure effective service delivery. Working collaboratively with health, housing, and community services to coordinate care. Maintaining accurate and up-to-date client records in compliance with local policies and legislation. Participating in team meetings and contributing to service development initiatives.To be successful in this role, you will need to possess the following essential qualifications and skills:
A recognized Social Work qualification (Degree in Social Work, DipSW, or equivalent). Registered with Social Work England. Experience in adult social work, particularly in community settings. Strong assessment and analytical skills. Excellent communication and interpersonal abilities. We're recruiting for a Transitions Social Worker in Stoke-on-Trent, working with young people (typically 14-25) as they move from Children's Services into Adult Social Care - a critical stage where your input directly shapes long-term outcomes.
🌟 The RoleYou'll play a key role in supporting young people with additional needs - including learning disabilities and complex needs - ensuring smooth, well-planned transitions into adult services.

✅ Essential Requirements (Non-Negotiable)Qualified Social Worker (Social Work England registered)Proven experience working as a Social WorkerExperience working with young people, transitions, or adult servicesStrong understanding of the Care Act, safeguarding and Mental Capacity Act🚫 This is not suitable for newly qualified or stepping-up candidates - you must be confident managing cases independently.
