We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Assessment Team in Liverpool. This pivotal role involves working with vulnerable adults to assess their needs, develop care plans, and ensure they receive the necessary support services. Your key responsibilities will include:

  • Conducting comprehensive assessments of adult clients to identify their needs and risks.
  • Developing and implementing person-centered care plans in collaboration with clients and their families.
  • Providing ongoing support and monitoring of client progress, making adjustments to care plans as necessary.
  • Collaborating with other professionals and agencies to ensure holistic support for clients.
  • Maintaining accurate and timely documentation in accordance with organizational policies and procedures.

To be successful in this role, you will need to possess the following essential qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work or equivalent).
  • Registration with Social Work England.
  • Proven experience in adult social work, particularly in assessment and care planning.
  • Strong communication and interpersonal skills, with the ability to build rapport with clients and their families.
  • Excellent organizational skills and the ability to manage a varied caseload effectively.
