As a key member of our Assessment Team, you will be responsible for:
Conducting comprehensive assessments of adult service users to identify their needs and strengths. Developing and implementing tailored care plans in collaboration with service users and their families. Providing ongoing support and advocacy to ensure that individuals receive appropriate services and resources. Maintaining accurate and up-to-date case records in compliance with local and national guidelines. Collaborating with multidisciplinary teams and external agencies to facilitate holistic care.The ideal candidate will possess the following essential qualifications and skills:
A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England (or relevant regulatory body). Experience in adult social work, particularly in assessment and care planning. Strong communication and interpersonal skills, with the ability to build rapport with service users.
