Are you a dedicated and compassionate social worker looking to make a meaningful impact in the lives of adults? Join our dynamic Assessment Team in Liverpool, where your expertise will contribute to the well-being and support of individuals in need. We are seeking a Qualified Social Worker to assess, plan, and implement care strategies that promote independence and enhance quality of life for our service users.

As a key member of our Assessment Team, you will be responsible for:

Conducting comprehensive assessments of adult service users to identify their needs and strengths. Developing and implementing tailored care plans in collaboration with service users and their families. Providing ongoing support and advocacy to ensure that individuals receive appropriate services and resources. Maintaining accurate and up-to-date case records in compliance with local and national guidelines. Collaborating with multidisciplinary teams and external agencies to facilitate holistic care.

The ideal candidate will possess the following essential qualifications and skills:

A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England (or relevant regulatory body). Experience in adult social work, particularly in assessment and care planning. Strong communication and interpersonal skills, with the ability to build rapport with service users. Excellent organizational skills and the ability to manage a diverse caseload effectively.

Join our dynamic Assessment Team in Liverpool as a Qualified Social Worker specializing in Adult Services. We are seeking a dedicated professional who is passionate about making a difference in the lives of vulnerable adults. In this role, you will conduct assessments, develop care plans, and ensure that individuals receive the support they need to lead fulfilling lives. Your expertise will play a crucial role in safeguarding and promoting the welfare of adults in our community.

Your key responsibilities will include:

Conducting comprehensive assessments of adults to determine their needs and eligibility for services. Developing and implementing person-centered care plans in collaboration with clients and their families. Providing ongoing support and monitoring of clients’ progress, adjusting care plans as necessary. Working closely with multidisciplinary teams to ensure holistic service delivery. Maintaining accurate and up-to-date case records in accordance with regulatory standards.

The ideal candidate will possess the following qualifications and skills:

A recognized Social Work qualification (BA/MA in Social Work or equivalent). Current registration with Social Work England. Proven experience in adult social work, particularly within assessment and care planning. Strong communication and interpersonal skills, with the ability to build rapport with clients and stakeholders. Demonstrated knowledge of relevant legislation and best practices in adult safeguarding.
