We are seeking a dedicated and experienced Senior Social Worker to join our Hospital Team in Wakefield. You will work collaboratively with healthcare professionals to provide comprehensive assessments, interventions, and support to patients and their families within a hospital setting.
Key responsibilities will include:
Conducting thorough assessments of patients’ social, emotional, and environmental needs. Developing and implementing care plans in partnership with patients, families, and multidisciplinary teams. Providing crisis intervention and support to patients and their families during hospital stays. Facilitating discharge planning and ensuring continuity of care post-discharge. Advocating for patients’ rights and access to necessary resources and services. Supervising and mentoring junior social work staff and students.The ideal candidate will possess the following essential qualifications and skills:
A recognized qualification in Social Work (e.g., BA or MA in Social Work). Current registration with the relevant professional body (e.g., Social Work England). A minimum of 3 years’ experience in social work, preferably within a healthcare or hospital setting. Strong assessment, communication, and interpersonal skills. Ability to work effectively in a fast-paced environment and manage competing priorities.
