Join our dedicated team in Birmingham as a Qualified Social Worker specializing in Discharge to Assess/Community. In this rewarding role, you will play a crucial part in supporting individuals transitioning from hospital to home, ensuring they receive the care and support they need to thrive in their communities. Your expertise will be vital in assessing needs, coordinating services, and working collaboratively with multidisciplinary teams to promote positive outcomes for service users.
Your key responsibilities will include:
Conducting assessments of individuals’ needs and identifying appropriate community resources. Developing and implementing care plans in collaboration with service users and their families. Facilitating timely hospital discharges while ensuring a seamless transition to community-based care. Monitoring and reviewing care plans to ensure ongoing effectiveness and responsiveness to changing needs. Providing guidance and support to families and caregivers throughout the discharge process.The ideal candidate will possess the following qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current registration with Social Work England. Experience in hospital discharge planning and community social work. Strong communication and interpersonal skills, with the ability to build rapport with diverse populations.
