We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work Hospital Team in Yorkshire. This dynamic role offers the opportunity to make a meaningful impact on the lives of individuals and families within a hospital setting. You will work collaboratively with healthcare professionals, patients, and their families to ensure that social care needs are effectively identified and met, promoting well-being and positive outcomes.

Your key responsibilities will include:

Conducting assessments and developing care plans for patients with complex needs. Providing emotional support and guidance to patients and their families. Collaborating with multidisciplinary teams to ensure holistic patient care. Advocating for patients’ rights and access to necessary services. Maintaining accurate records and documentation in accordance with policies and regulations.

The ideal candidate will possess the following essential qualifications and skills:

A recognized social work qualification (e.g., CQSW, DipSW, or equivalent). Registration with Social Work England. Proven experience in adult social work, particularly within a hospital or healthcare environment. Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals. Excellent organizational skills and the ability to manage a varied caseload.
