We are seeking a dedicated and compassionate Social Worker to join our Learning Disabilities team in Swindon. As a Social Worker in our Learning Disabilities team, your key responsibilities will include:
Conducting assessments to identify the needs and strengths of individuals with learning disabilities. Developing and implementing person-centered care plans in collaboration with clients, families, and multidisciplinary teams. Providing ongoing support and advocacy to clients, ensuring access to appropriate services and resources. Facilitating communication and cooperation between clients, families, and external agencies. Maintaining accurate and up-to-date case records and documentation in accordance with statutory requirements.The successful candidate will possess the following essential qualifications and skills:
A recognized Social Work qualification (e.g., BA/MA in Social Work). Current registration with Social Work England. Experience working with individuals with learning disabilities or in a similar field. Strong assessment, communication, and interpersonal skills.
