We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work - Community Team in Oxfordshire. As a key member of our team, you will be responsible for providing high-quality social work services, promoting independence, and enhancing the well-being of individuals and families.
Your key responsibilities will include:
- Conducting comprehensive assessments of service users’ needs and risks.
- Developing and implementing care plans in collaboration with service users and their families.
- Providing ongoing support and guidance to individuals, ensuring they have access to necessary resources and services.
- Working closely with other professionals, agencies, and community organizations to facilitate integrated care.
- Maintaining accurate and up-to-date case records and documentation in accordance with local policies and procedures.
To be considered for this role, you must possess the following essential qualifications and skills:
- A recognized social work qualification (Degree in Social Work or equivalent).
- Registration with Social Work England.
- Experience working with adults in a social care setting.
- Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
- Excellent problem-solving skills and the ability to work independently and as part of a team.
