We are currently seeking a dedicated and experienced Qualified Social Worker to join our team in Lambeth, specializing in supporting individuals with learning disabilities and autism. As a key member of our team, you will play a crucial role in providing high-quality care and support to vulnerable individuals in our community.
Key responsibilities and duties:
- Assessing the needs of individuals with learning disabilities and autism, and developing tailored care plans to support their well-being and independence.
- Working collaboratively with multi-disciplinary teams, including healthcare professionals, to ensure holistic care and support for service users.
- Conducting regular reviews and assessments to monitor progress and make necessary adjustments to care plans.
- Advocating for the rights and best interests of individuals with learning disabilities and autism, ensuring their voices are heard and respected.
- Providing guidance and support to families and carers, offering advice on coping strategies and accessing additional services.
Essential qualifications and skills:
- Qualified Social Worker with a valid registration with the Health and Care Professions Council (HCPC).
- Experience working with individuals with learning disabilities and autism, preferably within a community setting.
- Strong assessment and care planning skills, with the ability to work collaboratively with diverse teams.
- Excellent communication and interpersonal skills, with a compassionate and empathetic approach to supporting vulnerable individuals.
- Ability to work effectively under pressure, prioritize workload, and meet deadlines.
