We are currently seeking a dedicated and experienced Adult's Social Worker to join our OPPD Team in Lambeth. As a member of this dynamic team, you will play a crucial role in providing support and intervention to vulnerable adults in the community.

Key responsibilities and duties:

  • Conducting assessments and developing care plans for adults with complex needs
  • Working collaboratively with other professionals and agencies to ensure holistic support for service users
  • Providing advocacy and support to empower individuals to make informed decisions about their care
  • Monitoring and reviewing the effectiveness of interventions and making adjustments as necessary
  • Participating in multi-disciplinary meetings and case conferences to coordinate care provision

Essential qualifications and skills:

  • Qualified Social Worker with relevant registration and experience in adult social care
  • Demonstrated knowledge of relevant legislation and policies governing adult safeguarding and support services
  • Strong assessment and analytical skills with the ability to make sound decisions in complex situations
  • Excellent communication and interpersonal skills to engage effectively with service users, families, and colleagues
  • Ability to work autonomously and as part of a team in a fast-paced and challenging environment
