We are currently seeking a dedicated and experienced Qualified Social Worker to join our Reading team, specializing in Physical Disability and Older People. As a key member of our team, you will be responsible for providing high-quality social work services to individuals with physical disabilities and older people in the community.

Key responsibilities and duties:

Assessing the needs of individuals with physical disabilities and older people to develop care plans and support packages. Providing advocacy and support to clients to ensure their rights and choices are respected. Collaborating with healthcare professionals, families, and other agencies to coordinate holistic care for clients. Participating in safeguarding procedures and taking appropriate actions to protect vulnerable individuals. Maintaining accurate and up-to-date records of interventions and progress.

Essential qualifications and skills:

Qualified Social Worker with a recognized social work qualification and registration with the relevant regulatory body. Demonstrated experience working with individuals with physical disabilities and older people in a social work setting. Strong assessment and care planning skills with the ability to work autonomously and as part of a multidisciplinary team. Excellent communication and interpersonal skills to engage effectively with clients, families, and partner agencies. Understanding of relevant legislation, policies, and procedures related to social work practice in the field of physical disability and older people.
