We are currently seeking a dedicated and experienced Qualified Social Worker to join our Westminster - QSW - South Complex Care team. As a key member of the team, you will be responsible for providing high-quality social work services to individuals with complex care needs.

Key responsibilities and duties include:

Conducting comprehensive assessments to identify the needs of service users Developing and implementing care plans in collaboration with multi-disciplinary teams Providing ongoing support and intervention to promote the well-being and independence of service users Advocating for the rights and best interests of vulnerable individuals Participating in case conferences and reviews to ensure the effectiveness of care plans

The ideal candidate will possess the following essential qualifications and skills:

Qualified Social Worker with current registration with the Health and Care Professions Council (HCPC) Demonstrated experience working with individuals with complex care needs, preferably in a similar setting Strong communication and interpersonal skills, with the ability to build rapport with clients and colleagues Excellent assessment and analytical skills to identify needs and develop appropriate interventions Ability to work effectively in a fast-paced and challenging environment, prioritizing workload and meeting deadlines

Additional requirements for this role include:
