We are seeking a dedicated and experienced Qualified Social Worker to join our Long Term Support & Review team in Buckinghamshire. The successful candidate will be responsible for managing a caseload of clients, conducting assessments, and providing ongoing support to ensure that individuals receive the care and services they need.

Key responsibilities include:

  • Conducting comprehensive assessments of clients’ needs and circumstances.
  • Developing and implementing personalized support plans in collaboration with clients and their families.
  • Regularly reviewing and updating care plans to reflect changes in clients’ situations.
  • Working closely with multidisciplinary teams, including healthcare professionals and community organizations.
  • Maintaining accurate and timely case records in compliance with statutory and organizational guidelines.

Essential qualifications and skills required for this role include:

  • A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Current registration with Social Work England.
  • Proven experience in adult social work, particularly in long-term support and review.
  • Strong assessment and analytical skills, with the ability to develop effective care plans.
  • Excellent communication and interpersonal skills, with a compassionate approach to client care.
