We are seeking a dedicated and experienced Qualified Social Worker to join our Long Term Support & Review team in Buckinghamshire. This is an exciting opportunity to make a meaningful difference in the lives of adults within the community. The successful candidate will be responsible for managing a caseload of clients, conducting assessments, and providing ongoing support to ensure that individuals receive the care and services they need.

Key responsibilities include:

Conducting comprehensive assessments of clients’ needs and circumstances. Developing and implementing personalized support plans in collaboration with clients and their families. Regularly reviewing and updating care plans to reflect changes in clients’ situations. Working closely with multidisciplinary teams, including healthcare professionals and community organizations. Maintaining accurate and timely case records in compliance with statutory and organizational guidelines.

Essential qualifications and skills required for this role include:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current registration with Social Work England. Proven experience in adult social work, particularly in long-term support and review. Strong assessment and analytical skills, with the ability to develop effective care plans. Excellent communication and interpersonal skills, with a compassionate approach to client care.
