We are seeking a dedicated and compassionate Qualified Social Worker (QSW) to join our South Complex Care team in Westminster. This role is crucial in delivering high-quality support and services to individuals with complex care needs. As a QSW, you will work collaboratively with multidisciplinary teams to assess, plan, implement, and review care packages that promote independence and enhance the quality of life for our service users.

Key responsibilities include:

  • Conducting comprehensive assessments of service users’ needs and circumstances.
  • Developing and implementing care plans in collaboration with service users, families, and other professionals.
  • Monitoring and reviewing care plans to ensure effectiveness and compliance with relevant legislation.
  • Providing support, guidance, and advocacy to service users and their families.
  • Maintaining accurate and up-to-date records in accordance with statutory requirements.
  • Participating in team meetings and contributing to service development initiatives.

To be successful in this role, you must possess the following essential qualifications and skills:

  • A relevant social work qualification (e.g., Degree in Social Work or equivalent).
  • Registration with Social Work England.
  • Experience in working with individuals with complex needs, including mental health issues, disabilities, or other vulnerabilities.
  • Strong assessment and analytical skills.
  • Excellent communication and interpersonal skills, with the ability to build rapport with diverse clients.
  • Knowledge of relevant legislation and best practices in social care.
